physical health, withdrawal risk, medication response, and overall clinical stability.

health staff, and program leadership to coordinate care.
● Communicate clinical findings, treatment recommendations, medication changes, and safety concerns
clearly and promptly to the appropriate members of the treatment team.
● Coordinate referrals, laboratory work, drug screening, specialty care, and transitions of care as clinically
indicated.
● Support safe medication administration, prescription verification, medication reconciliation, and appropriate
medication-management processes.
● Document assessments, diagnoses, treatment decisions, medication changes, client interactions, and care
coordination accurately and promptly in the electronic medical record (EMR).
● Participate in treatment planning, clinical consultation, team meetings, family collaboration, and other
interdisciplinary activities as appropriate.
● Educate clients regarding diagnoses, medications, treatment recommendations, recovery supports, and
available services to promote informed participation in care.
● Recognize and respond appropriately to urgent medical or psychiatric concerns and coordinate escalation of
care when needed.
● Contribute to program development, quality improvement, clinical workflows, staff education, and
initiatives that strengthen client care and organizational effectiveness.
● Maintain client confidentiality, professional boundaries, and respectful, trauma-informed care consistent
with WisHope policies and applicable professional standards.
